Narrative

An employee was using a pneumatic butterfly valve to mix flour and sugar when the flour stopped feeding from above. The employee was attempting to clear the flour when the pneumatic valve closed on the employee's right-hand fingers, resulting in a fingertip amputation and a right ring finger amputation.
