Narrative

An employee was handling a racking board section. When the employee knocked a pin out of the racking board section, the racking board fell and hit the employee's left leg. The employee suffered a dislocated hip and fractured pelvis, requiring hospitalization. The racking board was not properly tied off at the time of the incident.
