Narrative

A valve was installed on a sewer line. When the line was repressurized, a hose with a brass fitting came off the line and struck an employee in the face. The employee suffered a fractured jaw, a broken nose, shattered cheekbone, loss of teeth, and cuts and abrasions to the right eye.
