Narrative

An employee was backing into an industrial lift on the main floor with a cart of books. The lift was not on the main floor and he fell 12 to 13 feet down the shaft, with the cart of books landing on top of him. He suffered bleeding on the brain and a broken shoulder, and he required stitches to the back of the head and hospitalization.
