On November 7, 2018, at approximately 6:15 p.m., an employee was working with a coworker to troubleshoot a counterbalanced forklift. The employees were working on the engine, but immediately prior to the accident had trouble getting the transmission to disengage. They were standing opposite of each other on either side of the forklift with one hand on the ignition switch key and the other in the engine compartment. The forklift's transmission was left in gear and safety switch was disconnected or otherwise not functional to prevent unexpected startup and rolling. Suddenly, the forklift started to roll a few feet and struck a nearby feeder machine staged on saw horses that had some plastic and aluminum material placed on it. Upon impact, that material shifted counterclockwise and struck the injured employee, crushing and pinning him against the forklift. He suffered a fractured pelvis.
