Narrative

An employee was conducting maintenance on a belt and pulley system on a scrap conveyor. He was tracking the belt. He had loosened the head pulley and was retightening it when his hand slipped into the running belt and pulley system. His right index finger was amputated up to the first joint. The system was unguarded at the time of the incident.
