A truck had dumped a load of scrap metal, and a large piece of pipe rolled under the tandem tire. An employee was standing to the side of the truck (about 15 feet away) as it started to pull away. The tire then rolled over the pipe, causing it to shoot out from under the tire and strike the employee in the shin. The employee suffered a broken leg.
