Narrative

An employee was relieving the pressure on a tank lid. He pulled the first and then second latch to let the pressure release, then hit the third latch with his foot. The third latch opened and the lid struck him, knocking him up and over the railing onto the ground below. He was hospitalized for a fractured wrist and spleen and back injuries.
