Narrative

An employee was unloading a bulk trailer. He pressurized the trailer and heard a hissing sound. He climbed up on top of the trailer to investigate what was leaking. He discovered the cleanout hatch was hissing. At that time, he touched the cap and it flew off, striking him in the forehead. He was hospitalized with a forehead laceration and required stitches.
