Amputations

An employee was aligning a casting body that was not centered. The employee was using shims and the tailstock to align the casting body. While holding some pieces of shim material on the center, the employee activated the foot pedal to move the tailstock forward to hold the casting body. The foot pedal malfunctioned and did not move the tailstock forward. When the employee looked at the foot pedal, the tailstock engaged, moved forward, and the employee's fingers were caught between the casting body and the tailstock. The right index and middle fingers were amputated halfway through the fingernails.
