Narrative

An employee was opening a 15' wide by 20' tall manual overhead door using the pull chain. While using the pull chain to the pulley and sprocket, a spring mechanism fell and struck him on the head. The employee was hospitalized with a fractured skull and a cracked C1 vertebrae.
