An employee was reversing a forklift truck down an inclined and elevated driveway when the forklift fell upright off the elevated driveway, which was approximately 16 inches above level ground. The employee's right hand was crushed between the cab of the forklift and a guardrail post, breaking the right middle finger and tearing tendons.
