Narrative

An employee was trying to repair a machine used to flip kettles. A screw had broken in the pivot block, causing a kettle to get stuck; the employee was using a hoist to lift the kettle to reduce tension in order to remove the kettle. The kettle fell onto his hand, amputating the tip of his right thumb.
