Narrative

A maintenance technician was replacing a 65-inch motor belt in a grinder mixer. The belt was installed behind a gear box that drove two chains via sprockets. The employee's right arm was inside the motor compartment when it suddenly started to turn. One of the chains amputated his right arm after wedging it against the sprocket.
