An employee was working at a well site where tubing was being pulled. As a block was being lowered to place a tube, the brake handle broke, allowing the block to fall down to the decking where the employee was working. The employee was struck by the block or other materials falling from about 6 to 10 feet above. He suffered a head laceration.
