Narrative

A welder was welding a nipple to a tank when a flame shot from the tank opening and caused second degree burns to another employee's chest. The injured employee was hospitalized. There may have been some residual cleaning product left after cleaning the oil from the cut (2-inch hole) where the weld was going to be made.
