Narrative

At 6:45 p.m. on March 27, 2019, an employee was operating a stand-up forklift in reverse. As the forklift approached a stack of filters, the employee's left little finger was crushed between a pallet in the filter stack and the vertical safety bar on the forklift. The finger was broken and part of it was amputated.
