Narrative

An employee was in a dry laundry area packing line. When the employee opened a door to adjust a rail, a chain and sprocket caught the employee's left hand, causing cuts and amputations to the index finger (tip amputation), middle finger (amputation to the second knuckle), and ring finger (amputation from tip to nailbed).
