Narrative

An employee was on an A-frame ladder while trying to loosen a nut from the top of the boom arm of a car wash. The employee was putting force on the wrench when the ladder tilted to its side and the employee fell five feet to the ground. The employee suffered a broken collar bone, nine broken ribs, and a punctured lung.
