Narrative

An employee noticed that a pallet slipped on the shelving while he was centering a pallet below. The pallet shifted and fell on top of a box that was on the lower pallet. The employee's finger was pinched between the box and the lower pallet. The employee sustained a partial amputation of the left middle fingertip.
