Narrative

An employee was standing on top of a conveyor belt that feeds the main bowl in the concrete plant trying to adjust the pulley. He was trying to loosen clamps or bolts when the belt started and the employee's right leg was caught in the belt opening resulting in amputation of the employee's right foot.
