Narrative

An employee was delivering pool supplies. The rope used to lift the roll-up door on the back of the truck got stuck. The employee attempted to use the powered lift gate to get the rope out by operating the lift gate and caught their left index finger between the lift gate and the truck. The finger was amputated.
