Narrative

On May 30, 2019, at approximately 7:40 p.m., an employee was performing set-up tasks on a material molder when the employee's right hand contacted an unguarded rotating cutter head. The injury resulted in an amputation to the right index finger at the second joint, and a partial amputation to the right middle fingertip.
