Narrative

An employee was attempting to reposition a board on a stacker unscrambler when the unscrambler moved and caused another board to fall on the employee's hand. The employee's hand was caught between the two boards. The employee sustained a crushing injuring to the right middle finger, resulting in amputation. The machine was not locked out at the time of the incident.
