Narrative

On June 12, 2019, at approximately 10:06 a.m., an employee was working near the back of a concrete truck when the bolts that hold the tag axle in place broke, causing the tag axle to fall and strike the employee. The employee was hospitalized with a broken clavicle, broken foot, broken ribs, and a ruptured spleen.
