Narrative

On August 12, 2019, at approximately 9:15 AM, an employee was troubleshooting the press stacker when the employee's right hand entered the stacker and activated the lift plate. The plate came down on the employee's right-hand fingers, resulting in a partial amputation to his middle and ring fingers.
