Narrative

An employee was loading a boom attachment onto the tines of a forklift. Noticing that the chain was wrapped around the clamp, the employee attempted to unwrap it. When the chain released, the employees right index finger was caught in the slack of the chain; a bone was broken and the tip was amputated.
