Narrative

At about 6:45 a.m. on August 16, 2019, a temporary employee was using an electric powered industrial vehicle to pick cases in a warehouse. The employee's left foot was pinned and crushed between the vehicle and a racking support. The employee was hospitalized, undergoing surgery.
