Narrative

An employee was on a ladder inspecting the coil section of an air handling unit when a PVC cap with a steel bushing burst free and struck the employee in the lower right jaw, knocking him off the ladder. He fell backward onto concrete, causing a 1-inch laceration to the back of his head. The impact to his face caused his jaw to break.
