Narrative

An employee was standing on an 8-foot A-frame ladder, cutting exhaust hood ductwork apart for removal. A portion of the ductwork fell and struck the ladder, causing the employee to lose his balance and fall to the floor. The employee suffered a head injury and a broken collarbone.
