Narrative

At about 10:15 a.m. on September 26, 2019, an employee was helping to set a manhole section on a site. The hook keeper on the chain sling broke, causing the manhole section to drop. The employee's left index finger and thumb were pinned between two manhole sections, suffering severe trauma.
