Narrative

An employee was working as a catcher, approximately 30 feet from the lumber molder machine. While the machine was idling, the employee was cleaning up behind the machine using an air hose and nozzle. At some point, the bottom head of the lumber molder machine activated and amputated the employee's right thumb and index finger from the middle knuckle up.
