Narrative

During unloading of a commercial water heater from a delivery truck, an employee's left index finger was pinched between a fork on the unloading vehicle and the top edge of the water heater. The left index finger was lacerated approximately 1 inch from the tip. A portion of the left index finger was surgically amputated.
