The first piece did not fully go inside the second, causing the nylon lifting straps to loosen and making adjustments necessary. As the employee made adjustments, the 6,000-pound piece suddenly dropped into the other piece; the nylon straps tightened on the lifting bail ring and crushed the employee's fingers.
