Narrative

Employees were installing a 54-inch pipe. Both the manhole and pipe were inside a trench box. While operators were repositioning, the excavator slid down in the mud, causing the pipe to move further into the manhole. An employee was pinned between the interior wall of the manhole and the pipe. The employee was hospitalized with leg and abdomen bruising.
