The injured employee was steadying an 8-inch pipe that was positioned on a pipe stand so that another employee could grind a tack weld on the pipe. The pipe slipped off the stand and crushed the injured employee's left arm against the stand. The injured employee sustained a broken ulna and radius and required surgery.
