Narrative

An employee was driving an order picker forward between two racks when a rack guide rail broke, causing the vehicle to stop suddenly. The employee hit the end of the next piece of the guide rail and was thrown forward. His head hit the windscreen and he fell backwards. The employee sustained a concussion and a cut on the bridge of his nose.
