Narrative

An employee had operated a meat grinder with a foot pedal. Afterwards, the pedal was not put away in the assigned spot at the machine. The employee then started cleaning tasks and tripped over the foot pedal, falling to the floor and fracturing his left knee. He was hospitalized and had surgery.
