Narrative

An employee was making a jig to accommodate tube bending. The employee was holding stock near the chuck of the tube bending CNC machine while waiting for another employee to find an extension to hold the stock in the chuck, prior to actuating the clamp via a control panel. The clamp actuated, catching the employee's finger and degloving the fingertip which resulted in a medical amputation.
