Narrative

An employee was trying to flush contaminants from a chemical line. The flow came out at a higher rate than expected. Toluene di-isocyanate splashed out of the containment bucket and onto two employees. One of them, splashed in the head, face, and chest area, suffered skin irritation and the effects of chemical inhalation.
