Narrative

An employee was checking the fit of a pipe wrench on a rotating shaft when the pipe wrench became caught on the pipe and rotated, striking the employee. The employee then fell approximately 5 to 8 feet to the ground. The employee was hospitalized with vertebral and head injuries.
