Narrative

An employee was watching foam blocks travel down a conveyor. A piece of plastic that was on one of the blocks got wrapped around one of the conveyer's rollers. The conveyor stopped and the employee tried to remove the piece of plastic; the conveyor started again and the employee's hand was entangled in the plastic and pulled into the roller. The employee suffered a dislocated left shoulder and elbow.
