Narrative

An employee was getting ready to unload a truck. He extended the dock plate and was walking onto the truck when the truck driver pulled away to square the truck up, causing the dock plate to fall. The employee fell to the ground, hitting his head and landing on his back. He sustained contusions to his back and the back of his head.
