Narrative

At approximately 3:00 a.m. on January 31, 2020, an employee was operating a delivery truck's lift gate when the employee fell forward off the lift gate after miss-stepping over the edge. The employee fell chest-down on the ground and was hospitalized with four fractured ribs.
