Narrative

An employee was unloading concert equipment using an automatic loader. The garage door on a ramp would not open; while the employee was trying to open it automatically, the automatic loader slipped, catching his right foot between the ramp and the I-beam underneath the garage door. His right second toe was medically amputated.
