Amputations

An employee was filling the cup and lid dispensing mechanism of a packing machine when the lids became jammed. She was able to remove all lids that were stacked in the dispensing mechanism, but one lid was jammed between the stacking mechanism and the lidding stomper. As she attempted to remove this lid, her right index fingertip was pinched between the lid stomper and the rotating table (nest plates), resulting in the amputation of the fingertip right above the nail bed.
