Narrative

An employee was reaching with a push stick to adjust an empty plastic bottle that was stuck about 14 feet overhead. A forklift backed out of an 18-wheeler trailer and struck the employee's right leg, breaking it. The employee was hospitalized and the employee's right foot was medically amputated.
