Narrative

At approximately 2:00 p.m., an employee working in the warehouse bent over to pick a piece of paper off the floor. At that time, a forklift was backing off from a trailer. As the forklift turned to the right, it made contact with the employee. The employee sustained foot fractures and was hospitalized.
