Narrative

An employee was changing out a broken cable. The cable was raised and had a punch installed to hold the weight of the door while the employee was performing the maintenance. When the cable was replaced, the weight of the door dropped, resulting in an amputation of the employee's left middle finger to the joint and a damaged ring finger.
