Narrative

An employee was carrying a 6-foot-long and 5-inch-diameter pipe filled with dirt when he tripped into a 2-foot-deep ditch. He fell on top of the pipe with his hand between the pipe and ground. He sustained a fracture to his right hand that required surgery.
